Thank you for your commitment to working with children, students & vulnerable adults at First Church and those of the community. This packet will need to be updated every 4-5 years per the state. Because we want to provide children, students & vulnerable adults with a safe environment as well as comply with the PA State law, we ask that you complete the following forms and place them in the mailbox of Erin Mowrey or mail them to:

If you own an equivalent to any of these clearances or trainings, you may submit a copy of the certificate to the Church Office as a substitute.
There are three parts that need to be completed in order to volunteer or work at First Church:
1) SAFE MINISTRY POLICY
You can find the form here. Please complete, sign and return to the church office.
2) CLEARANCES
A) PA CRIMINAL RECORD CHECK
Please complete and return the form to the church office.
B) PA CHILD ABUSE HISTORY CERTIFICATION
Please complete, sign and return the form to the church office. Once this is processed, you will receive an official copy from the state in the mail which will need to be brought into the church office for us to make a copy.
C) FBI FINGERPRINTING
Follow the instructions on this form. Once this is processed, you will receive an official copy from the state in the mail which will need to be brought into the church office for us to make a copy.
3) SAFE MINISTRY VIRTUAL TRAINING
To complete this section, you will need to contact the Church Office to receive a link in order to fulfill the video trainings from Praesidium Academy. Once you have created an account, you will need to complete these two sessions, every year:
A) DUTY TO REPORT: MANDATED REPORTER (Spanish or English)
This will teach you the basics and what is required of a mandated reporter. Once you have completed the training, please notify the Church Office. This will take approximately 30 minutes to complete in its entirety.
B) FOUNDATION: PREVENTING ABUSE IN YOUTH-SERVING ORGANIZATION (Spanish or English)
This will explain the tactics of abusers, and specific steps you can take to prevent abuse. Once you have completed the training, please notify the Church Office. This will take approximately 45 minutes to complete in its entirety.
There may be other videos required for your ministry area. Please check in with your ministry staff member in case more training is needed.
PLEASE NOTE: Any payments you make out of pocket can be reimbursed by the Church. Please submit any receipts to the Church Office as soon as you can upon completing these items.
