Because we want to provide children, students & vulnerable adults with a safe environment as well as comply with the PA State law, we ask that you complete the following forms and place them in the mailbox of Erin Mowrey or mail them to:

3) PA CRIMINAL CLEARANCES
A) PA CRIMINAL HISTORY
Please complete and return to the church office. You can find directions on completing the form here.
B) PA CHILD ABUSE HISTORY CLEARANCE
Please complete, sign and return this form to the church office. Once this is processed, you will receive an official copy from the state in the mail which will need to be brought into the church office for us to make a copy.
C) FBI FINGERPRINTING
Follow the instructions on this form. Once this is processed, you will receive an official copy from the state in the mail which will need to be brought into the church office for us to make a copy.
PLEASE NOTE: Originals from the PA Child Abuse and the FBI Fingerprinting will be mailed to your home. Please bring them to the office as soon as you receive them and we will make a copy for your file, as your clearances are not considered complete without them.
